Our Mission
Our Mission is to end homelessness in North Sonoma County. Programs to provide opportunities and support for clients to work toward self-sufficiency, independence and permanent housing.
OUR MISSION:
Ending homelessness in Northern Sonoma County.
By engaging with community members who are struggling with or at risk of losing housing, we help transform lives with action plans for their success. Our programs promote self-sufficiency, independence, and permanent housing. Through hope, heart, tenacity, and synergy, we strive for dignity for every member of our community.
CORE VALUES:
Heart
Empathy and care drives all we do for each other and our community. Our compassion is grounded in authenticity, core boundaries, and we strive to meet people where they are with an open mind believing every human being has a right to their dignity.
Hope
Regardless of the circumstance, we believe change and growth is possible for every person, organization, and community. We have a responsibility to our own self-care in order to continue to offer hope to one another and people we serve.
Tenacity
In spite of choice and barriers, we strive to provide opportunities and equity for our participants. We do what we say and are consistent with our services. We are accountable to one another and our stakeholders.
Synergy
We recognize that our work is complex, and no single person or organization can do this work alone. We work in concert with each other, our participants, and our community partners to end homelessness in Northern Sonoma County.
OUR PURPOSE:
The purpose of Reach for Home’s programs is to provide opportunities and support for participants to work toward self-sufficiency, independence and permanent housing. Participants team with a case manager to help access resources and services that will help them make necessary changes in their lives. Furthermore, our program offers participants the opportunity to gain self-confidence and to become self-sufficient. We assist participants that live in Windsor, Healdsburg, Cloverdale and Geyserville.
